We are currently hiring for a Family Support Specialist in Albion, NY.  The Family Support Specialist is responsible for the use of person-centered practices in assisting individuals through OPWDD eligibility, intake for Arc of Genesee Orleans Services, transition from school processes and other tasks associated with the administration of agency Family Support Contracts and Services. 

Duties and Responsibilities:

  1. Upholds and promotes the Mission, Vision, and Values of The Arc of Genesee Orleans.
  2. Conducts intake interviews, secures necessary documentation for OPWDD eligibility, submits eligibility packets for consideration and provides assistance to individuals and their families through intake process as needed.
  3. Completes central intake responsibilities for individuals interested in agency services, secures necessary documentation and distributes referrals to Program Directors and/or their Designees.
  4. In collaborations with individuals, their families, advocates and appropriate stakeholders, facilitates   and/or assists in the development of person-centered plans, Individualized Education Plans and transition
  5. portfolios.
  6. Provides technical assistance on issues related to eligibility, Individualized Education Plans, community
  7. supports, intake and transition to individuals with disabilities, family members and professionals.
  8. Confers with outside agencies and programs to become acquainted and informed concerning resources that will promote successful supports and services within school and community settings.
  9. Assist in the development of proposals/plans that will demonstrate quality improvement and offer innovative methods in service delivery for students in their transition years.
  10. Provides specialized information, referral, and training services relating to laws and regulations that affect people with disabilities.
  11. Comply with all documentation and reporting requirements by agency and funding sources.
  12. Performs any other duties as deemed necessary by Project Coordinator, Director of Community Services and the Executive Director.
  13. Conducts self in such a manner as to meet agency policies and standards defined in The Arc of Genesee Orleans Staff Member Handbook at all times.
  14. Ability to work in a variety of settings that include; office, homes of individual's served, school districts and community organizations.

Hours:  40 hours per week

Pay:  $13.98 per hour plus full-time benefits
