We are currently hiring for a Support Broker in our Community Services department.   

Main Function: The Support Broker is to maintain the Self Directed Service (SDS) Plan and Budget.  This includes working with the SDS participant and Circle of Support (COS) to review and update the SDS Plan/Budget every six (6) months, and as often as needed, so that it meets the needs of the individual and remains current and eligible for Medicaid funding, and attending the participant's Individualized Service Plan (ISP) review, SDS Plan/Budget review and COS meetings as requested.

Job Duties: 

  1. Assisting the SDS participant to develop and maintain the circle of support. This includes assisting the SDS participant to identify members, to identify and assign roles and tasks for circle members, to schedule circle of support meetings, ensuring that the SDS participant is aware that circle meetings must occur at least every three months; and assisting the participant to facilitate circle of support meetings.  The broker assists with the overall functioning of the circle of support, ensuring that members are freely chosen by the participant and that the circle is operating in the participant's best interest and within the principles of self-determination.
  2. Providing education and training to the participant and his/her circle of support in implementing the SDS Plan/Budget according to Medicaid and New York State standards, and providing other technical supports as needed to keep the SDS Plan/Budget functioning effectively.
  3. Monitoring SDS expenditures to ensure that expenditures do not exceed the SDS budget by assisting the participant to review the Monthly Spending Detail Report provided by the Fiscal Intermediary (FI) and working with the participant and circle of support to ensure monthly spending is within budget.
  4. Assisting the SDS participant to review, revisit, and update his/her person-centered plan on a regular basis.
  5. Assisting the participant to properly document SDS services according to Medicaid and OPWDD regulations and policy. This includes assisting the SDS participant to review and submit to the FI employee time sheets, the monthly summary note, mileage and expense reimbursement forms, and all other required documentation.  Timeliness is necessary to ensure Medicaid compliance and for billing purposes.
  6. Assisting the participant to hire and retain appropriate support staff. This includes assisting the SDS participant in recruiting, interviewing, hiring, scheduling and supervising self-hired staff as well as assisting the individual to identify and retain adequate back-up staffing to ensure availability as needed by the participant.
  7. Working with the individual, the COS and the Medicaid Service Coordinator (MSC) to help identify and develop connections in the community that support aspects of the SDS Plan/Budget.
  8. Other duties as assigned by the Fiscal Intermediary Manager or his/her designee.
  9. Establishing and maintaining good working relationships and open communication with the participant's Medicaid Service Coordinator (MSC), Fiscal Intermediary (FI), and Developmentally Disabled Regional Office (DDRO) SDS Liaison.
  10. Ensuring that there is a Support Broker Agreement in place that is consistent with the approved SDS Plan/Budget.
  11. All Support Brokers are required to meet and maintain OPWDD broker training and certification requirements in order to provide broker services. Brokers must send training certificates to the DDRO SDS Liaison to get credit for training.  Brokers are not paid from SDS Plans to attend training or update certification.


  1. AAS Degree plus one year experience working with people with a developmental disability.
  2. Current NYS Drivers' License with an acceptable driving record as determined by the agency (local travel is required).

Location:  Genesee and Orleans County

Hours:  40 hours per week.  Must have flexibility with schedule.

Rate:  $14.00 per hour plus full-time benefits.
